Thomas Pynchon a nudist? You guys really do cover it all. By the way, best example of a milieu story really is "The Lord Of The Rings." Tolkien really did approach it by creating his own world first and then manufacture stories to tell within that setting. The milieu/setting is a character in the story itself.
